BUSINESS MEMBER APPLICATION
New business members or renewing businesses must download and complete the application and waiver and release form. Annual membership dues are $50 for businesses. Please mail the completed application, signed waiver and release, a business card and your check to WSATVA at P.O. Box 4283, Grand Junction, CO 81502-4283. Or, you can bring cash or check to the next membership meeting.
Note: We require a business card so we can put it up on our website and link it to your business, as well as publish it in our newsletter.
